Checking Insurance Benefits & Script

This page will guide you through checking your nutrition counseling benefits to determine whether our time together is covered by your insurance plan. 

We are in network with United Healthcare (UHC) PPO. Coverage varies from plan to plan and it is up to you to know your benefits.

If you have a different insurance plan, we can provide you with a monthly “superbill” upon request, which can be submitted to your insurance company for reimbursement. A superbill is a receipt for your nutrition counseling sessions, which you can submit to your insurance company for potential reimbursement depending on your individual coverage and plan. Sessions will be paid at the time of service and the reimbursement from your insurance will be paid directly to you. 

Below are some questions to ask your insurance company to see if you have out-of-network benefits for nutrition counseling. 

Call the member services or customer support number on the back of your insurance card and ask: 

  1. Does my plan cover out-of-network benefits for outpatient nutrition counseling (CPT Codes: 97802 and 97803)?

    • If yes, how many sessions are allowed per plan year?

    • Do I have preventative benefits for nutrition counseling?

    • Does my plan cover diagnosis code Z71.3 with CPT code 97802? If yes, how many sessions are allowed per plan year?

  2. Does my plan only cover medical nutrition therapy visits that are considered “medically necessary”?

  3. Do I have a deductible to meet first? 

    • If yes, how much is my deductible? 

  4. Is telehealth covered for nutrition counseling (all sessions are via HIPAA compliant telehealth)

  5. Do I need a physician referral? If so, the physician referral must be brought to the initial appointment or emailed.

  6. What is my co-pay or co-insurance amount for outpatient nutrition counseling?

  7. My dietitian provides superbill receipts for our sessions. How do I submit these for reimbursement, and how long does the process take to receive financial return?

Write down the representative’s name and a reference # when checking your benefits for your records. 

Clients can pay for our services with cash, check, credit/debit cards, flex spending (FSA) cards or with Health Savings Account cards! A credit/debit card on file is required to secure appointments.
